Roster Signing Forms

This season, we are required to collect the Waiver of Liability and Consent to Treat forms from each player prior to roster signing. Click on the links below to download the forms.These forms need to be filled out by all travel team players (write player level at the top of the form). Note that it is not mandatory that the Medical History portion of the Consent to Treat form needs to be completed. Forms can be dropped off at Pagel, attention to the MYHA Registrar, prior to or at the first day of tryouts. Squirt players should have theirs turned in by the first day of the Squirt Development Program. The mailing address for the Pagel Activity Center is 18313 Highway 7, Minnetonka 55345.
