MYHA Participation Rule and Waiver Requirements - Updated for the 2011-12 Season

2011-12 Minnesota Hockey Participation and Waiver Policy

To be eligible to play with MYHA, you need to either live in the Minnetonka school district or go to school within the school district boundaries. 

If you participated with MYHA in 2010-11 and completed a waiver form and had no change of address or schools you do not need a new waiver form this year.
 
If you are a first time player not residing in the Minnetonka School district 276, but attending school within the district, you may choose to participate in MYHA.  To do so, you will need to provide a waiver to MYHA that issigned by the hockey association and the governing hockey district where you reside.
 
The waiver form is a four part carbon form provided by Minnesota Hockey and cannot be obtained electronically. You may pick up a blank form at Pagel, or email your address to Anne Madson at registrar@tonkahockey.org and a blank form will be mailed.
